MS benefits in Serbia: disability, care allowance, parking and free medication

What you are entitled to with an MS diagnosis in Serbia and how to claim it: free DMTs through RFZO, care allowance, disability pension, parking card, tax relief and protection at work.

Free DMT medications through RFZO — the most important right

I keep returning to this subject because a useful answer should be precise without losing sight of everyday life. People with multiple sclerosis in Serbia have the right to free disease-modifying therapies (DMTs) through the Republic Health Insurance Fund (RFZO). In June 2021 the number of patients receiving therapy was increased from 2,500 to 3,500 patients ([Mondo.rs](https://mondo.rs/Magazin/Zdravlje/a1475753/multipla-skleroza-lekovi-o-trosku-drzave.html)).

Which medications are covered:

First-generation drugs (interferon beta-1a, beta-1b, glatiramer acetate) — moderate efficacy. High-efficacy drugs (ocrelizumab, ofatumumab, natalizumab, cladribine, fingolimod and others) — for patients with active disease. A referral is issued by a neurologist specialising in MS. RFZO approves therapy based on diagnosis, disease activity and failure of previous therapy.

> Tip: Keep all documentation — MRI reports, relapse confirmations, referrals. RFZO sometimes requests additional documentation for approval of high-efficacy drugs. If your referral is denied, you have the right to appeal and request a reassessment.

Disability and pension insurance

MS can be recognised as a basis for disability under the Serbian Pension and Disability Insurance Act. Assessments are carried out by medical commissions of the Social Insurance Institute (PIO).

Types of disability:

Working capacity — reduced ability to perform your previous occupation. General working capacity — ability to perform any work. Actual working capacity — how much the patient can work in real conditions.

Important: The Serbian disability assessment regulation is applied strictly by disease name, which often makes it difficult for people with MS to obtain disability status because MS is not explicitly listed in all categories ([DisabilityInfo.me](https://disabilityinfo.me/ljudska-prava/item/2538-oboljeli-od-multiple-skleroze-tesko-ostvaruju-pravo-na-invalidninu-pravilnik-ih-ne-prepoznaje)). That is why it is crucial that the neurologist describes functional limitations (walking, vision, balance, cognition) in detail rather than relying on the diagnosis alone.

Disability pension depends on length of service and degree of disability. Patients without work experience may be entitled to a cash allowance under special regulations.

Care assistance — help from another person

MS patients who cannot perform basic life activities (bathing, dressing, feeding, moving) are entitled to care assistance from another person.

Referral is issued by a general practitioner or specialist (neurologist, physiatrist). A commission of the Social Insurance Institute assesses the degree of need for help. Cash allowance is paid if care is provided by a family member (usually a spouse or parent) or a caregiver can be hired. The amount depends on the degree of impairment and the average salary in Serbia.

> Practical tip: Apply as soon as the need arises. Documentation of functional limitations (EDSS score, physiatrist report, family statement) helps with the commission.

Parking card for people with disabilities

People with recognised disability in Serbia are entitled to a free or discounted parking card that allows parking in reserved spaces and free parking in public areas.

- The request is submitted electronically via [eUprava](https://euprava.gov.rs/usluge/7075) or in person at the municipality (depending on local government).

- A verified health booklet and disability certificate are required.

- A parking space for people with disabilities can also be requested at residential buildings (with the consent of the assembly of tenants).

Tax relief and other financial rights

Tax exemption: People with disabilities and their parents/spouses can claim various tax reliefs under the Personal Income Tax Law:

Increased personal deduction for people with disabilities. Additional deduction for unemployed spouses providing care. Exemption from inheritance and gift tax for property transferred to a person with disabilities.

Other financial rights:

Student scholarships — students with disabilities are entitled to special scholarships and university fee exemptions. Free legal aid — in civil and administrative disputes, through the Free Legal Aid Bureau. Flat adaptation subsidies — local governments occasionally offer subsidies for adapting flats for people with disabilities (ramps, rails, bathroom).

Rehabilitation and balneotherapy

MS patients are entitled to physical therapy and rehabilitation through health insurance:

- A referral for physical therapy is issued by a neurologist or physiatrist.

- A certain number of treatments per year are covered within RFZO (depending on the insurance plan).

- Spa treatment — MS patients can exercise their right to rehabilitation in specialised spas (e.g. Sokobanja, Niška Banja, Vrnjačka Banja) through a specialist referral.

- Medical aids — crutches, walkers, wheelchairs, toilet aids — are partially or fully covered through the RFZO list with a physiatrist referral.

Workplace protection and employee rights

The Law on Prevention of Discrimination against Persons with Disabilities protects employees with MS from dismissal and discrimination.

Dismissal due to illness or reduced productivity caused by MS is prohibited (except in cases where the job position is no longer needed, following proper procedure). Right to adjusted working conditions — flexible working hours, remote work, adapted workstations, rest breaks. Disability insurance through the employer — the PIO fund covers part of the costs if the worker has to switch to lighter work due to illness. Trade unions — union membership can help advocate for the rights of workers with MS.

Patient rights and advocacy

Commissioner for the Protection of Equality — handles complaints of discrimination against people with disabilities, including refusal of medication approval by RFZO ([Commissioner — case 913-24](https://ravnopravnost.gov.rs/rs/913-24-republicki-fond-za-zdravstveno-osiguranje/)).

Patient organisations:

- Multiple Sclerosis Society of Serbia — education, support, connecting patients, advocacy ([multiplaskleroza.org.rs](https://multiplaskleroza.org.rs/)).

- MS Platforma Srbije — national organisation, member of the European MS Platform (EMSP), working to improve the position of people with MS.

- Local organisations — regional groups and initiatives providing psychological support, group therapy and education.

Summary of the most important steps

1. Verify your health booklet — without a verified booklet you cannot access RFZO services.

2. Request a referral for DMT therapy from an MS specialist neurologist.

3. If you have functional limitations, request a disability assessment from your general practitioner.

4. If you need help with daily activities, apply for care assistance.

5. Request a parking card via eUprava or your local government office.

6. Use your right to rehabilitation — physical therapy and spa treatment are covered.

7. Find out about tax reliefs at the local tax administration or with your accountant.

8. Join a patient organisation — support and information about changes in regulations.

Serbia has a developed support system for people with multiple sclerosis, but many rights remain underutilised due to lack of awareness of procedures and bureaucratic obstacles. The key is to be persistent, document everything, not give up on appeals, and use the support of patient organisations. Your neurologist, general practitioner and patient associations are your allies in exercising the rights that belong to you by law.
